The chain walks off the sprocket under load
We checked the sprocket alignment and tension by hand, but the chain still walks off after a few hard starts. What should we inspect next?

We had this when the plates flexed just enough under load to move one sprocket out of plane. A stiffer spacer and checking alignment while the wheels were pushed against the floor fixed it for us.
Confirmed it was frame flex. Added support near the axle and it stays put now. Marking this solved for anyone who finds the thread later.
